Forms & Regulations
for Local Chapters
This page is intended to help the members in downloading all the
necessary forms and regulations for opening or renewing a local chapter.
Please make sure to follow the guidelines and notify the Units Affairs
Officer as mentioned in the
Open or Activate an
ESANA's Local Chapter page.
The election committee should file the required forms
and send them (both electronically and hardcopy) to ESANA Units officer at:
